“锁定”与“回拉”:医疗保险制度对流动人口居留意愿的影响

“Lock-in” and “Pull-back”: The Impact of Medical Insurance on the Residence Intention of Floating Population

摘要: 基于2017年全国流动人口动态监测调查数据,实证分析了医疗保险制度对流动人口居留意愿的影响。研究发现:在居住地参保能够提高流动人口的本地居留意愿,具有“锁定”效应,而在户籍地参保则会降低流动人口的本地居留意愿,具有“回拉”效应;同时,“回拉”效应与“锁定”效应均随医疗保障制度待遇水平的提高而增强。进一步分析表明,医疗保险对流动人口居留意愿的影响受流动距离的制约,居住地参保的“锁定”效应和户籍地参保的“回拉”效应均随流动距离的增加而增强。政府应继续深化医保制度体制机制改革,简化异地报销程序,提高异地报销水平,打破地域区隔的制度藩篱,实现流动人口公平享受医疗保障权益。

Abstract: Based on China Migrants Dynamic Survey data (2017), this paper empirically analyzes the impact of medical insurance on the residence intention of floating population. It is found in the study that participating in the medical insurance in their current residence has the “lock-in” effect and increases the residence intention of floating population, while participating in the medical insurance in the place of domicile has the “pull-back” effect and reduces the residence intention of floating population. The “pull-back” effect and “lock-in” effect are strengthened with the improvement of the benefits level of medical insurances. Further analysis shows that the impact of medical insurance on the residence intention of floating population is restricted by flow distance. The “lock-in” effect of the insured in the place of residence and the “pull-back” effect of the insured in the place of domicile both increase gradually with the extension of flow distance. The government is supposed to continuously deepen the reform of medical insurance, simplify the reimbursement procedures in different places, improve the level of off-site reimbursement, and break the system barriers of geographical division to realize equal access to medical security rights by floating population.
